This statement relates to the processing of your personal data and medical data by Echocentrum Almere. We endeavor at all times to treat your data confidentially, to adequately secure it and to only use it in accordance with our privacy and data security policy, which complies with the General Data Protection Regulation (GDPR). You agree to this storage and processing by having your care carried out by our practice.
Collection of data
In order to provide you with good care, we need certain personal information from you. It concerns:
- Personal data: data that can identify a person.
- Medical data: data that is important to provide adequate care
For the purpose of the services of Ultrasound Center Almere, the following data is stored and processed digitally:
- Name, address and residence details
- BSN number
- Date of birth
- Gender
- Health insurance
- Name of your partner
- Correspondence address
- Phone number and e-mail address
- (Ultrasound) photos aimed at medical treatment.
- Ultrasound reports
We process your data on the basis of the treatment agreement as described in the Medical Treatment Agreement Act (WGBO).
Purpose of data recording
The main purpose of data registration by the practice is to provide the best personal care and related services. Secondary objectives are to answer questions about your specific situation, to send information regarding the period of your pregnancy and delivery and to handle the financial settlement of the care provided with other (care) institutions.
Exchanging personal information
We only pass on your data to third parties if this is necessary to provide good care. If specific permission is required for this based on the law, we will ask you for this in advance.
There are some parties that process your personal data for different purposes. However, we always remain responsible for the processing of your data and we have therefore concluded processing agreements with various parties. The practice has a register of processing agreements that can be consulted.
However, the GDPR includes the possibility to process medical data without your consent, if this is necessary to 'ensure high standards of quality and safety of healthcare'. We do inform you that your data is processed by these authorities.
The authorities with which our practice can share (part of) your data are:
- Orpheus:
Orfeus is the software with which we digitally organize, maintain and consult your file (containing personal and medical data). - Care domain:
Zorgdomein is a digital platform to better connect healthcare providers and clients. Consider matters such as referrals, requesting diagnostics, online consultation between healthcare providers, access to the care offered, etc. - Care email:
Zorgmail is a secure email function for safely and confidentially exchanging personal and medical data in healthcare. - Peridos:
Peridos is the national digital file in which healthcare providers record data in the context of screening for Down syndrome and structural ultrasound examination (SEO). Data to improve and optimize the quality and primary process of screening. - Vecozo:
Vecozo plays a social role in cost control in the Dutch healthcare sector. With the digital communication point, they focus on reducing the administrative burden of all chain parties in healthcare.
We also work with various healthcare providers from maternity care. You can think of:
• The GP (you must inform him or her that you are pregnant)
• The gynaecologist
• Other healthcare providers with whom you could be receiving treatment
Retention periods
We do not process your data for longer than is necessary to provide good care. This also applies to data we receive from third parties. All personal data will be kept for the period required by legal provisions. For example, the file is subject to a legal retention obligation of 15 years.
Rights of data subjects
- You can view your personal information. Please send us a written request.
- If you find that the data we have stored about you is incorrect, you may ask us in writing to adjust or supplement your data.
- You can ask us to delete your personal data. We will comply with this, unless this conflicts with other legal obligations. If we do this, we may no longer be able to provide you with responsible care. The file obligation requires a retention period of 15 years. Therefore, in that case we store your data in an inactive archive, invisible and inaccessible to the normal user.
- You can limit or withdraw your consent to our processing of your personal data. We may then no longer be able to guarantee responsible care. In that case, we store your data in an inactive archive (see above).
- You can submit a request to us to transfer your personal data to another (healthcare) institution.
- You can object to us if you do not agree with the way we process your personal data in your specific situation.
- You will receive a message from us if something has gone wrong with your personal data.
Birth announcements
Clients often send us a birth announcement after the birth of their child. We really appreciate this and are happy to hang it in our waiting room. By choosing care from Ultrasound Center Almere, you give permission for this. If you would rather not have the birth announcement hung here, please let us know.
Complaint
If you have a complaint about the processing of your personal data, we request that you contact us. We will then do our utmost to find a satisfactory solution together with you. If this does not work, you have the option to file a complaint with the Dutch Data Protection Authority, the supervisory authority in the field of privacy.
Cookies
The Echocentrum Almere website uses cookies. These are intended to collect statistics about the use of the site and to easily make connections to other platforms that may be of interest to our clients, such as Facebook. If a user does not want to participate in this, he or she can block the use of cookies on the first visit or via the browser settings.
